Have this ready
- Your legal business name exactly as it appears on your formation document, and your federal EIN.
- The authorized representative's contact details and title.
- Your entity type, state of formation, and principal business address.
- Any professional or financing licenses held by the business or its owners.

You will not upload documents on this page. After your application is reviewed and accepted, you will receive partner-portal access and upload your supporting documents there, through a secure channel.
What submitting means
Submitting this application does not create a partner relationship, guarantee acceptance, or authorize any activity on the platform. If accepted, a separate Partner Agreement must be executed before activation.
